“Let’s Talk Story,” an interactive program with Kōkua Mau, Hawai‘i’s Hospice and Palliative Care Organization, will help people start thinking and talking about how they make decisions, who should be involved, and what types of medical care they want and don’t want. It will also cover basics of advance care planning and an overview of the Advance Health Care Directive (AHCD) and  POLST (Providers Orders for Life Sustaining Treatment).

Notary services will be available at the end of the presentation. RSVP required. Call the library by Friday, January 31st  to register.

Kōkua Mau is Hawai‘i’s ‘Movement to Improve Care’ and foster conversations. We are a network whose collaborative membership includes health plans, hospice providers, hospitals, palliative care experts, long-term care providers, educational institutions and passionate community champions. We provide public and professional education, networking for professionals, and public policy and advocacy.

If you need an auxiliary aid/service or other accommodation due to a disability, please contact the library at least 7 days prior to the program date. We will make every attempt to fulfill all requests for accommodations. All programs are subject to change. For a list of upcoming library events, visit www.librarieshawaii.org/events.
