Print single-sided. DO NOT SIGN THE FORM. Cross out any mistakes with a single line and write the correction nearby using BLACK INK. Do not use correction fluid or tape on the application.

Children Under the Age of 16
Children under the age of 16 must follow the above requirements along with the following:
1. Both parents with valid IDs must be present along with the child(ren). If only one (1) parent can appear in person, one of the following is required:
If a legal guardian is accompanying the child, a certified court order granting guardianship must be submitted.
2. Proof of Parental Relationship i.e. birth certificate indicating parents’ name(s) must be provided.
Minors Ages 16 and 17
Applicant ages 16 and 17 must appear in person with one parent to show parental awareness. If a parent cannot be present, the minor applicant must have a valid photo ID and present a form of parental awareness such as a signed statement of consent from a parent, or a parent’s check for the passport fee(s).
If your valid passport has been lost or stolen, you must submit “Statement Regarding a Lost or Stolen U.S. Passport Book and/or Card (Form DS-64)” along with “Form DS-11”and apply in person.
A separate check or money order is required for the execution fee and the passport book/card application fee. The current fees are as follows:

Regular walk-in passport service hours for Hawaii State Library are Mon-Fri, 10 am – 1 pm and Sat, 9 am-12 pm.
From February 4 to March 10, 2020, the Hawaii State Library will offer additional passport processing hours on Tuesdays from 10:00 a.m. 4:30 p.m. View/print flyer
The tickets for passport service are given out on a first-come-first-serve basis. Applicants are advised to arrive early, especially during the peak season, from January through April, when the tickets run out really fast.

Demand for passport service peaks from late December through May and during school breaks, and it may be necessary to limit the number of applications that we can process during our service hours. Call the National Passport Information Center (NPIC) 24/7 at 1-877-487-2778 (1-888-874-7793 TDD/TTY) to make an appointment at the Honolulu Passport Office if traveling within 14 days, or within 4 weeks if destination country requires a visa. An appointment and proof of travel are required. |
Adult passport renewal (DS-82) is done by mail. Hawaii State Library is unable to accept DS-82 Renewal Applications. If you qualify to use the DS-82 Renewal by Mail form, please mail it in. Do not come to the library.

If you would rather renew your passport in person at the library, you can do so by filling out Form DS-11 and paying the execution fee of $35 in addition to your passport renewal fee(s).
