First time applying for a U.S. passport?

We can help. The Hawaii State Public Library System offers passport services at participating libraries. We accept Form DS-11 U.S. Passport Applications only.


Who Should Use the Form DS-11?

Use Form DS-11 if any of the following are applicable:

  • You are a first-time applicant.
  • Your most recent passport was issued at age 15 or under.
  • Your most recent passport was issued more than 15 years ago.
  • Your current valid passport is damaged, lost or stolen.
  • You had a name change since your last passport was issued, but cannot submit a certified copy of the legal name change document.

Prepare for your visit

You will need five things to make your passport application process go smoothly:

  1. U.S. Passport Application Form DS-11: Enter your information and print out using the online Form Filler, OR print out a PDF copy of the form and fill out by hand.

Print single-sided. DO NOT SIGN THE FORM. Cross out any mistakes with a single line and write the correction nearby using BLACK INK. Do not use correction fluid or tape on the application.

  1. Proof of Citizenship: your certified birth certificate, consular report of birth abroad or naturalization certificate. If you have a previous U.S. passport but you are not eligible to renew it by mail using Form DS-82, bring your most recent undamaged, full-term U.S. passport.
  2. Valid Photo ID: Hawaii driver’s license, Hawaii state I.D., military I.D. or passport. If you present a temporary driver’s license, a driver’s permit or an out-of-state ID, you must present additional IDs.
  3. One (1) Passport Photo: 2”x2” color photo with a white background on professional photo paper, taken within 6 months. No glasses, no uniform or headgear. Hawaii State Library do not provide passport photo services.
  4. Two personal checks or money orders: No cash or debit/credit card payments are accepted.
    1. Execution fee of $35 per applicant payable to the Director of Finance – State of Hawaii. Multiple execution fees may be combined into one check or money order, e.g., $70 for two, or $105 for three applicants.
    2. Passport fee payable to the US Department of State. Multiple passport fees may be combined into one check or money order, e.g., $220 for two, or $330 for three adult passport book fees.

Additional Requirements for Minors

Children Under the Age of 16
Children under the age of 16 must follow the above requirements along with the following:

1.  Both parents with valid IDs must be present along with the child(ren). If only one (1) parent can appear in person, one of the following is required:

If a legal guardian is accompanying the child, a certified court order granting guardianship must be submitted.

2. Proof of Parental Relationship i.e. birth certificate indicating parents’ name(s) must be provided.

Minors Ages 16 and 17

Applicant ages 16 and 17 must appear in person with one parent to show parental awareness.  If a parent cannot be present, the minor applicant must have a valid photo ID and present a form of parental awareness such as a signed statement of consent from a parent, or a parent’s check for the passport fee(s).


Lost or Stolen Passport

If your valid passport has been lost or stolen, you must submit “Statement Regarding a Lost or Stolen U.S. Passport Book and/or Card (Form DS-64)” along with “Form DS-11”and apply in person.

A separate check or money order is required for the execution fee and the passport book/card application fee. The current fees are as follows:

* The U.S. Passport Card may only be used for re-entry into the United States at land borders and sea ports-of-entry when traveling from Canada, Mexico, the Caribbean and Bermuda. Passport cards may not be used for air travel.

Passport Service Hours

Regular walk-in passport service hours for Hawaii State Library are Mon-Fri, 10 am – 1 pm and Sat, 9 am-12 pm.  more info

Subject to demand and staff availability, it may be necessary to limit the number of applications accepted during the service hours on any given day.

Other Passport Acceptance Facilities in OAHU

Demand for passport service peaks from late December through May and during school breaks, and it may be necessary to limit the number of applications that we can process during our service hours. Need an Urgent Passport Services?

Call the National Passport Information Center (NPIC) 24/7 at 1-877-487-2778 (1-888-874-7793 TDD/TTY) to make an appointment at the Honolulu Passport Office if traveling within 14 days, or within 4 weeks if destination country requires a visa. An appointment and proof of travel are required.

Adult Passport Renewal (DS-82)

Adult passport renewal (DS-82) is done by mail. Hawaii State Library is unable to accept DS-82 Renewal Applications. If you qualify to use the DS-82 Renewal by Mail form, please mail it in. Do not come to the library.

You may use Form DS-82 if your most recent passport meets all five of the following requirements:

  • Your most recent passport is submitted with your application
  • Was issued within the last 15 years
  • Was issued when you were age 16 or older
  • Is undamaged
  • Was issued in your current name, or you are submitting an original or certified copy of your name change document, e.g., marriage certificate, divorce decree, or court order

If you would rather renew your passport in person at the library, you can do so by filling out Form DS-11 and paying the execution fee of $35 in addition to your passport renewal fee(s).
