“Let’s Talk Story About Advance Care Planning: It’s How We Care for Each Other”

Join us for a free public talk by Kokua Mau, a Hawaii Hospice and Palliative Care Organization,  that will discuss the importance of advance care planning – a way to communicate your wishes for care, and also for naming a substitute decision maker who can speak for you if you can’t speak for yourself.

Using materials from the The Conversation Project, learn how advance care planning impacts your health care not only for yourself, but also your family and loved ones.  By using these materials, you can start making a plan and sharing it with others.
